Patient Care Coordinator
Northwest Community Health Center is seeking a team oriented and dependable Patient Care Coordinator to support multidisciplinary teams of healthcare providers in a fast paced outpatient setting.
The successful candidate will work as part of the health care team by efficiently managing internal and external referral coordination, population health management for assigned teams, and health record preparation in advance of patient visits.
The position is responsible to ensure effective coordination of patient care to improve the patients understanding of the health care process and improve overall patient quality of care to be compliant with Patient Center Medical Home requirements.
Minimum qualifications include a High School Diploma, 1 year of experience working in a healthcare setting, and certification or licensure is preferred (CMA, RMA, or LPN), but not required.
Position is full time, and offers competitive wages and benefits.
